How to Nominate
To complete the online nomination form you will need the following information:
- Information about the person you are nominating including name, address, e-mail, and phone number.
- You must have available for upload the following documents regarding your nominee; Letter of Nomination, Bio, and up to two optional Supporting Documents.
- Nominator name, address, e-mail, and phone number. The nominator must be a member of USITT.
- The nominator must complete the Suggested Plaque Wording field (50 word limit). Wording will be subject to editing if deemed necessary. Deadline to nominate: July 6, 2020
- Once the application is started and at least one document is uploaded, the nominator will receive a confirmation e-mail with a link to get back into the nomination form to modify information. Through this link you can modify the information as often as necessary, please note that after July 8, 2019 the link will no longer be active.
